By signing below, I acknowledge and agree on behalf of myself and all members of my group that:

• Beauty & Spa On The Go may take photos and/or short video clips during our spa experience.
• These images and videos may be used for marketing purposes, including (but not limited to) social media, website content, online advertising, printed brochures, and promotional campaigns.
• I affirm that I have obtained verbal or written consent from all members of my group to participate and be included in photo/video documentation.
• No compensation is provided for participation, except the exclusive offer noted below.
• Beauty & Spa On The Go agrees not to disclose any identifying personal information (such as full names) without additional permission.

Exclusive Offer Acceptance

In appreciation for our participation in the above, we understand that Beauty & Spa On The Go is providing us with a special spa experience offer which may include, but is not limited to professional photographer, complimentary upgrades, value-added services, or a discounted group rate.

This exclusive offer is extended with the understanding that our group participation in photography/videography is granted.
